Please note that the Township of Asphodel-Norwood does not take appointments. You must call 249-494-5631 for assistance in booking your appointment. To attend a vaccine clinic, you must have an appointment. Walk-ins will not be accepted. View the full pre-clinic communication handout. Before you book an appointment

You should consult your healthcare provider before booking a vaccine appointment. If you do not have one, you can speak to one through Telehealth 1-866-797-0007 or Peterborough Virtual Care Clinic 705-536-0882.
Identify someone who can help you book your appointment. COVID-19 vaccine appointments are primarily booked online. If you need assistance with online booking please ask a family member or close friend to assist you.
Information you need to book an appointment:

your name and birthdate
postal code
Health card number on the front and back (if you have a red and white health card you will need to book over the phone)

Before booking an appointment, ensure you have access to to transportation on that day.
Once you are eligible to book a vaccine appointment you will continue to be eligible. If a clinic does not work for you now, you will be able to book at a later date for another clinic.

At this time, appointments are ONLY available for people born in 1966 or before

If you cannot book online, call 1-833-943-3900 for assistance with booking (open seven days a week 8:00 a.m. to 8:00 p.m.)

BEFORE YOU CALL, please be ready to provide your name, date of birth, availability for the appointment and Ontario health care number. First and second vaccine appointments will be booked at the same time.

If you are having issues with the booking system or have general COVID-19 vaccine questions please contact the Ministry of Health for assistance at 1-888-999-6488.

Before Your Appointment

Before you arrive, take the COVID-19 online assessment. If you do not pass the assessment, please do not attend the appointment. Call or go online to reschedule your appointment.
Arrive at the time your appointment is booked. If you arrive sooner you will be asked to wait outside or in your vehicle until space is cleared for you to come in.
Bring your Ontario health card.
Please follow the clinic rules posted.

Need Transportation?

Community Care Peterborough is pleased to offer transportation to those need of access to COVID-19 Vaccination Clinics. Please contact your local clinic to request a ride. You will have to provide your name, address, and vaccination appointment date/time. Based on available resources (volunteer drivers), Community Care will do its best to meet your needs. There will be a small fee charged (based on distance) and payable directly to the driver at the time of pickup.

Individuals requiring transportation do not need to register or be an active client of Community Care to receive this service.

Restrictions:

Open to adults only (must be 18 years of age)
No additional stops – transportation will be provided to the clinic and return home only.
